Insurance Info: We’re contracted with major Health insurance carriers, & for your convenience we also always check the benefits on your current plan. Please bring a valid picture ID & current insurance info that includes Medicare. If this is an auto or personal injury case, please bring in your auto med-pay, your attorney’s information, along with your valid picture ID & general insurance information and ID.

Self Pay: We accept Visa, MasterCard, Personal Check, & Cash.
